This lovely little item is something you get for completing the game at least once.
It can be used in the following locations the next time you play:

Roof of the Alternate School
Doors to Hospital prior to Moth boss
Inner area of the motel
The houseboat before Lighthouse
Top floor of the Lighthouse

I won't give away any more than that, 'cause I don't want to spoil the fun for you, but give those a try, in the order given, and see what happens!
